As a Disabilities Services Officer, you will play a vital role in supporting individuals with disabilities by providing high-quality services that empower them to achieve their goals and live independently. This role involves coordinating services, advocating for clients' needs, and working closely with families, carers, and other professionals to ensure the best outcomes for those we support.
Key Responsibilities:
- Provide person-centered support to individuals with disabilities, tailored to their unique needs and preferences.
- Develop and implement care plans in collaboration with clients, families, and other stakeholders.
- Coordinate services and referrals to ensure comprehensive support is provided.
- Advocate for the rights and interests of clients, ensuring they receive appropriate assistance and accommodations.
- Monitor client progress and adjust support strategies as needed.
- Maintain accurate records and documentation of services provided.
- Collaborate with multidisciplinary teams to deliver holistic care.
- Provide advice and support to families and carers regarding disabilities and available services.
Skill Requirements:
- Empathy and a genuine passion for supporting individuals with disabilities.
- Strong communication skills, both written and verbal, to effectively interact with clients, families, and professionals.
- Organizational and time management abilities to manage multiple tasks and meet deadlines.
- Knowledge of disability rights, services, and support systems (an advantage but not required).
